The High Risk Pediatric Clinic at Clinica Sierra Vista
Supporting Medically Fragile Children and their Families
This clinic is dedicated to improving the health and quality of life of
children with complex medical conditions. Our multidisciplinary team consists
of a pediatrician, nurse practitioner, social worker, referral clerk,
and medical assistants. Together, we provide comprehensive primary care
services, same-day appointments for sick visits, dental screening, and
care coordination services. We aim to support families and caregivers
of medically fragile children through linkage to appropriate community
resources.
Eligibility Criteria
Children 0-21 yrs. of age with:
A complex and chronic medical condition
Receiving multiple healthcare services
Dependence on life-sustaining technology (oxygen, CPAP, ventilator, G tube, etc.)
Caregivers/family members who have a high need for social services support
